Licensing Information

Special Markets requires that agents and agencies that are to receive commissions from us submit a completed W-9 form.

In order to write accounts through Special Markets Insurance Consultants, Inc., you and your agency must be licensed and appointed with the carrier.

Accident & Health

Our Accident Medical carriers require that agents and agencies soliciting business on their behalf be appointed with them.

Below is a list of requirements for their appointment process:

» Copy of agent's Accident & Health (A&H) license for the state in which the insured is located.
» Copy of the agency's Accident & Health (A&H) license for the state in which the insured is located or a copy of the state specific A&H license for the entity receiving commission.
» Copy of the E&O Declarations page.
» The original Producer Agreement / Appointment Data Sheet must be mailed into us, but a faxed version will be accepted until the original is received.

Property & Casualty

Special Markets requires that any agent soliciting General Liability business through us be licensed and appointed.

Below is a list of requirements for their appointment process:

» Copy of agent's Property & Casualty (P&C) license for the state in which the insured is located.
» Copy of the agency's Property & Casualty (P&C) license for the state in which the insured is located or a copy of the state specific P&C license for the entity receiving commission.
» Copy of the E&O Declarations page.
» The original Appointment Data Sheet must be mailed to us, but a faxed version will be accepted until the original is received.
