In order to write accounts through Special Markets Insurance Consultants, Inc., you and your agency must be licensed and appointed with the carrier.

Our Accident Medical carriers require that agents and agencies soliciting business on their behalf be appointed with them.
Below is a list of requirements for their appointment process:

| » | Copy of the agency's Accident & Health (A&H) license for the state in which the insured is located or a copy of the state specific A&H license for the entity receiving commission. |
| » | Copy of the E&O Declarations page. |
| » | The original Producer Agreement / Appointment Data Sheet must be mailed into us, but a faxed version will be accepted until the original is received. |
